BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/ bcr-2015-214303 DESCRIPTION A 33-year-old African-American man with a previous medical history of type II diabetes mellitus and tobacco abuse presented to our outpatient clinic because of a penile lesion. The lesion started 1 month prior to presentation. The patient reported a history of minor injury while zipping his pants, which resulted in slight redness at the tip of his glans penis, progressing to blackish discolouration. He was sexually active with one female partner and reported infrequent oral sex. He had no penile discharge, no pain and no urinary symptoms. His diabetes mellitus was diagnosed 3 years prior to presentation. He was non-compliant with oral hypoglycaemic medications. Examination showed normal vital signs and superficial dry gangrene with no drainage (figure 1). He had no inguinal lymphadenopathy. The reminder of his physical examination, including cardiovascular examination, was normal. Lab investigations showed haemoglobin A1c of 12.9% (reference 0–5.7%). Comprehensive metabolic panel, complete blood count and urinalysis were normal. The patient was started on insulin therapy and frequent local wound care with silver sulfadiazine. Two months later, his lesion totally resolved (figure 2). Haemoglobin A1c 4 months later was 5.8%. Penile gangrene that results from vascular trauma is a rare complication of diabetes mellitus. Penile calciphylaxis resulting from vascular calcification in patients with end-stage renal disease and diabetes is well established. Management of penile gangrene depends on the cause and includes conservative measures. Surgical intervention may be needed in patients who experience superimposed complications such as infections, extension of the gangrene or failure of conservative management.

Calciphylaxis leading to penile necrosis.
Penile gangrene is a rare disease. It represents a poor prognostic sign in end stage renal disease (ESRD) patients and an indicator of metastatic vascular calcification. Pathogenesis of this life threatening condition is not clearly understood and treatment is also controversial. We describe the case of a 61-year-old man known to have diabetes mellitus, chronic renal failure on haemodialysis, w...
متن کاملIschemic Gangrene of the Glans following Penile Prosthesis Implantation
The development of ischemic gangrene of the penis following implantation of prosthesis is unusual, and very few cases are available in the literature. As a result, no established treatment protocol is available. We report our experience within a case of gangrene of the glans following implantation of a three-component prosthesis. We present a 53-year-old male, smoker with diabetes and hyperchol...

متن کاملPenile Preservation With Subcutaneous Transposition During Fournier's Gangrene
A 50-year-old male with past medical history of diabetes mellitus presented with extensive Fournier's Gangrene. He had a wide-spread area of involvement and the wound vacuum placement involved the entirety of the phallus. We describe a surgical technique where the penis can be diverted from the site of the wound to allow for more secure wound vacuum placement and future reconstructive options.

Sir, With great interest, we have read the article by Liao et al. [1] entitled ‘Penile gangrene in a chronic dialysis patient’ in a recent issue of Nephrology Dialysis Transplantation. They reported a case of penile gangrene in a 66-year-old man with diabetic nephropathy in end-stage renal disease (ESRD) on regular haemodialysis for 1 year. The patient received partial penile resection to stem ...
